What it reads

A single Word table whose first row names its columns.

1. Last Name, First Name — printed large, swapped into reading order.
2. Designation — the middle line.
3. Service — the small line underneath.

One tent per page, landscape Letter. Fold each page along the centre line and the name reads the right way up from both sides.
